INVESTIGATIONAL NEW DRUG REVIEW PROCESS Investigational New Drug Review Process Prashanth Kumar Ponugoti Northeastern University INVESTIGATIONAL NEW DRUG REVIEW PROCESS Abstract Investigational New Drug (IND) review process begins from the time the sponsor files investigation new drug application and the purpose of the review is to confirm the safety and efficacy of the patient for the drug.Moreover in the process of review if the review committee notifies any deficiencies by the new drug then there will be a clinical however if there are no deficiencies the drug will be approved for new drug application and will be under continuous review to make sure , that the drug does not cause any potential harm. Applicant (Drug sponsor): An applicant or the sponsor is a person or a group who takes responsibility and initiates for the investigation of new drug.A person other than an individual that uses one or more of its own employees to conduct an investigation that it has initiated is a sponsor. The sponsor may be an individual, partnership, government agency, private agency, or any other organization and moreover the sponsor does not conduct the investigation process unless the sponsor is a sponsor-investigator. The applications are submitted to Center for Drug Evaluation and Research (CDER) Food and Drug Administration Document and Records Section 5901-B Ammendale Rd. Beltsville, Md. 20705-1266 II.Investigational New Drug Application: Sponsor thinks about new drug application when he has a successful results in preclinical studies (The studies conducted in animals to find the use of drug in human and ensure safety and efficacy) in other words it is the step taken after the successful preclinical studies to continue further studies that is clinical trial in humans. During the conduct of preclinical trial, step by step information of the process is recorded as data and stored. Thus stored information is helpful in filling an IND. The information is as follows A. Animal Pharmacology and toxicology studiesB. Manufacturing Information C. Clinical protocols and investigators information Animal Pharmacology and toxicology studies: The studies conducted in animals to ensure the use of the product is reasonably safe for initial testing in humans. Manufacturing Humans: This is the information obtained from manufacturing, storage, composition, storage and stability, for manufacturing of drug substance and produ ct. Clinical protocols and investigators information: protocols for proposed clinical studies to assess whether the initial-phase trials will expose subjects to unnecessary risks.Information on the qualifications of clinical investigators—professionals who oversee the administration of the experimental compound–to assess whether they are qualified to fulfill their clinical trial duties. The IND is not a marketing approval but The IND is the means through which the sponsor officially obtains this exemption from the FDA: however, its main intention is to detail the data that provide documentation that it is indeed reasonable to proceed with certain human trials with the drug. Types of INDs i. Investigator INDs ii. Emergency Use INDs and iii.Treatment IND After the submission of the IND to the Center for Drug Evaluation and Research (CDER) committee it checks thoroughly the medical, chemistry, toxicological, and statistical data INVESTIGATIONAL NEW DRUG REVIEW PROCESS The se terms are explained below in detail III. Medical Review: Medical also called as clinical review is conducted by medical officers or physicians to ensure the initial use of the drug in patients and are safe to use . This review is also conducted by the non medical officers based upon the data available from the re clinical studies.Medical review plays a major role in new drug review process and is an initiating step for the test of the drug in humans: however it is an important step to be conducted and based upon the results obtained from the review it has a further scope to apply for next steps that is to file new drug application. During this review process the medical reviewers or physicians evaluate the clinical trial protocol for to determine A. If the participants will be protected from unnecessary risks; and B. If the study design will provide data relevant to the safety and effectiveness of the drug.However the safety of the drug is taken by the sponsor during the phase I studies but during phase II, III, and IV Food and Drug Administration (FDA) must also ensure the safety of the medicament in humans for market approval. IV. Chemistry Review: Each review committee has an chemistry review department for reviewing the chemistry of the drug and the agenda of this chemistry department is to address issues related to drug identity, manufacturing control, and analysis. The reviewing committee ensures that the investigational new drug is adequately reproducible and stable.If the drug is neither adequately reproducible nor stable then the chances of approval for IND are less because they are not sure about the drugs action in the body, and don’t know what it does. So the major purpose of chemistry review is i. To ensure that the compound is adequately reproducible ii. To ensure that the compound is adequately stable In addition to this the sponsor should discuss any manufacturing and chemistry differences between the drug product proposed for the cli nical use and drug product used in animal toxicology studies.However these differences might affect the safety profile of the drug product and need to be stated if there are no differences. V. Pharmacology or Toxicology Review: This committee is a group of pharmacologists and toxicologists who evaluate the results of animal testing made during the clinical trial and compare it with the potential effects in humans. That is to ensure safety of the humans for the new drug. i. Pharmacology and Drug Distribution (21 CFR 312. 23(a)(8)(I)): This is nothing but absorption, distribution, metabolism and excretion of the drug in the animals.These results should be obtained by conducting them in animals, so it should be recorded with an example. Moreover this should not be a reason for the clinical hold because these rae the studies conducted in phase one of the IND and where FDA feels it as sponsor great responsibility factor for the drug in achieving safety and efficacy to the humans. ii. Tox icology Data (21 CFR 312. 23(a)(8)(ii)(a)): INVESTIGATIONAL NEW DRUG REVIEW PROCESS Toxicology data is required from both vitro and vivo studies, because particular studies depend upon nature of the drug and phase of human investigation.VI. Safety Review: After submission of the IND to the CDER then the committee takes about good 30 days to conduct review in every factor to achieve safety and efficacy and to avoid potential harm to the human. The review is an ongoing process however CDER notifies the sponsor immediately if there are any potential high risks associated with the drug upon usage in humans. If a notification is sent to sponsor then it is clear that the application is put on hold and the sponsor should conduct enough studies to clear the hold.This hold will be removed and allows the sponsor for further studies only when changes made to the clinical hold achieve the safety and efficacy and does not case potential risks. If the sponsor is not notified with any information from CDER from the day of IND submission to the 30 th day then on the 31st- day further study can be proceeded as submitted. VII. Clinical Hold Decision: The Clinical hold is issued in during the thirty day review period and this is due to the following factors 1. The drug causes potential harm to the humans 2.If drug is not having enough stability 3. The IND does not contain sufficient information required under 312. 23 to assess the risks to subjects of the proposed studies 4. The plan or protocol for the investigation is clearly deficient in design to meet its stated objectives. And CDER can hold the studies during the initial phases of the studies that is when filed for IND if observes potential risks on the data submitted initially. The Clinical hold will be removed if the sponsor addresses the issue based on the hold before the order is removed.In addition the clinical holds are reviewed by higher authorities of CDER to assure scientific quality and consistency in the Center's clinical hold decisions. Power in Sociology Essay

Sociology is a study of the development, structure, and functioning of human society. Sociology also studies the sophisticated connection between human behavior and individual life changes. Sociology examines by the way of social structure such as various social institutions that affect human attitudes, organizations and social categories. However, in the subject of Sociology of Work, it has combined sociology with work to improve the human behavior in the organization. According to Grint (2005), â€Å"Work occupies a substantial proportion of most people’s lives and has often been taken as a symbol of personal value†. He perceived that work will affect an activity that transforms nature which could be undertaken in social circumstance. However, power is the main topic to discuss in this essay. Power is the major explanation to the study of social society. According to J. G. March (1966 cited in Shafritz, Ott & Jang, 2005), it is not even related to organization and also among the community of decision making, business behavior, and small discussion. Meanwhile, power is not only used in the organization but theoretical able to apply in the other circumstances. However, the discussion in this essay will be critically evaluate the power that is used by those top level of the organization to control and possess to those lower level in the hierarchy. Coercive power is one of the powers that will be discussed in the argument, follow by social power and also legitimate power. This essay will include some of the theories by Weber and Marx to support the discussion of power. Bureaucracy means the top of the management holds the coordination and possession of the hierarchy and who devised a system that includes the rule and regulation to restrict the lower level subordinate in order to achieve organization goals, (Grint 2005). In the theory of Bureaucracy by Max Weber, he perceived that there are possess and control towards the lower level subordinate by the top in the organization. Weber has conceptualized of authority on bureaucracy by the sense of legitimate power and it will be the center of the organization’s processes. Weber also emphasizes rationalization process that regard to work organization and also able to improve development of technological and scientific thinking. This process would remind the employees to use the most appropriate way to achieve task base on the procedures and the formalize roles. Besides, according to Shafritz, Ott & Jang (2005), â€Å"The principle of office hierarchy and the level of graded authority mean a firm ly ordered system of super and sub-ordination in which there is a supervision of the lower office by the higher ones†. It means that the hierarchy and the level of an organization had their own system of authority to supervision the employees by the top of the management. Capitalism is devised by a German philosopher named Karl Marx. It is related to the industrial sociology and labor process. Marx had mentioned that industrial society had to be compared with agrarian society and the success of human freedom eventually, but the main reason of the social formation was only caused by the capitalist pattern and not the industrial process. Marx had emphasized that capitalism is more important than industrialism because it is the only capitalism. He also argued that â€Å"human species is different from all other animal species, not because of its consciousness but because it alone produces its own means of subsistence† (Grint 2005). On the other hand, alienation and exploitation still an important part of the capitalism. The impact on Marx’s theory in the modern industrial had strictly been use in the labor process. This impact had developed a perspective with the employment relations and employee behavior in the work design and also the organization. Alienation is the relationship between human expression and labor changes, and not using labor to fulfill their needs. Marx also stated that worker are alienates from their labor because it is no longer belong to the worker, but rather to the capitalist. In the system of capitalist, the minority owns the production, where the labor power is only owned by majority (Grint, 2005). Thus, the profit of production is through a commodity market and this is called alienation of production. Exploitation is a social relation on which capitalism is built. Worker pays less of wages than what they produce. It means that the worker gets a 10 dollars wages a day, he/she must produce more than 10 dollars a day worth of value. As we know, power mostly uses to control the hierarchy by the top of the management in an organization. Power in an organization has really big skill, whoever having the higher position in the organization will obtain the possession authority to control the entire organization. According to Giddens (1979 cited in Grint 2005), he argued that too many power which had been used at the same time would cause the least of the result eventually. It means that if the organization had too many of work relationship, it would be more complicated and uncontrollable by the management. However, coercive power is one of the powers that always uses by an organization. This power generally uses in some â€Å"low trust† organization to possess their worker and employees. Due to that, coercive power is often used by the top level to direct control the lower ones. Coercive power tends to be highly prescribed procedures which the workers are ordered to follow step by step of the job. Workers are in close supervision and under surveillance by the top level along the working period. Therefore, low commitment and low trust culture would be integrated into the coercive power regulated organization. Not even that, a tightly bureaucratic structure and culture will infuse in this coercive organization. Bureaucracy management is slightly similar to coercive powers which both are using force and impersonal forces to regulate the organization. Moreover, the internalization of an organization would be affected directly to the performance and productivity of an organization. I n other words, social power means internalization; basically it is a central element in analyzing human society and the way it is organized. Based on organization point of view, the strength of social power is defined as the potential ability of A to influence to B in the organization. According to French and Raven (1959 cited in Shafritz, Ott & Jang, 2005), power of a person is measurable by the influence ability although he may choose to use lesser than it. Due to that, internalization and social power are mostly the same, they are both study about process of acceptance of a set of norms and values established by people or group which are influential to the individual through the process of socialization. However, legitimate powers occur when a leader is being selected to an authority position. It means that the leader with legitimate power has the authority to issue order which they in turn have an obligation to accept. According to French and Raven (1959 cited in Shafritz, Ott & Jang, 2005), they said that the speculation and investigation about social specified behavior has been considerable, which is particularly pr escribed to give a position. People with legitimate power are usually involved in certain title or position. The impact of this legitimate power could affect the whole organization. At the higher hierarchy management person, such as manager and Chief Executive Officer (CEO), they are the main person who makes decision in the whole organization. In term of leadership, the top management would use legitimate power to force the employee to do anything they want and complete the task. But, the down-line employees could reject the forces by their up-line manager. According to Linton (1945 cited in Shafritz, Ott & Jang, 2005), the differences of the group norm are pursuant from whether they are alternative, culture and specialties. As far as concerned, Marx and Weber both have interpret the same meaning related to enforcement in the organization. In the above discussion, Weber has conceptualized of authority on bureaucracy by the sense of legitimate power. Which is means bureaucracy and legitimate power are similar concept of management way for a organization. He also devised that there are possess and control towards the lower level subordinate by the top in the organization. Legitimate power used by some of the organization to regulated their employee. The organization structure and performance will be improve and also increase productivity. In a legitimate organization, the top management has the authority to control the lower ones. However, coercive power also same as the legitimate power use manage the hierarchy in an organization. Coercive power is suitable to use in industrial base organization to supervise the worker since there is â€Å"low trust† structure in the organization. Both of these powers are slightly similar to bureaucracy management that uses to dominate the lower level employees in an organization. For example, Just in Time system (JIT) able to improve behavior of the worker and power capacity. JIT is mainly use by industrial sector in Japan which is produce just in time for them to use. JIT is a simple production method for manufacturing. It is to produce and deliver the product at once it is finished to be sold which is also called just in time to be sold. Lastly, there is another power relate to hierarchy management from top to lower level. Social power as we know as internalization of an organization, it is a study of someone’s influential to individual through the process of socialization. Social power is much more important for the hierarchy management . This power would be need more skill and knowledge when use to apply on employees. Top management need to know how to influence the lower ones to change their mindset. The worker will perform better and efficacy when their mindset is just toward the organization. Total Quality Management (TQM), the system that use in many organization since 1980s. In this case study of TQM, they had question toward power that to be capability of individual or group. They research on the structure and inter-personal characteristics within the bank division and branches that under same organization. From analysis of this case study, the power relation not only apparent in social power but also in coercive and political power. In conclusion, the power that discuss above which use to improve hierarchy management and that able to develop the human behavior in the organization. Power is the major explanation to study if social society. The three powers that had discussed in the above paragraph include coercive, legitimate and social power. All of these powers have enough use to handle the hierarchy management in the organization. However, coercive and legitimate power is more toward impersonal and forces way of management. Top management can use social power to influence their employees to work and change their mind of thinking in term any affection in the organization.

This speaks to extensive inefficiencies in the industries’ HRM, which have not only resulted in poor employee engagement, but widespread gaps in labour-related issues within the two industries (Nickson 2013, p. 6). For instance, in an endeavour to streamline their processes, most tourism and hospitality businesses hire additional employees, including seasonal staff, to ensure that each employee concentrates extensively on a single, minute task and executes it within minimal mistakes. Although this HRM strategy may prove effective in enhancing diverse aspects of the services provi ded by the industries’ employees it also enhances employee disengagement in their work. Employees typically require the opportunity to start at the bottom and climb the corporate ladder while also gaining experience. Therefore, repetitiveness is a major demoraliser. A lot is required from businesses in the tourism and hospitality industries to retain their employees. However, their HRM strategies are inefficient, and this contributes significantly to the industries’ unique issue with turnover. However, to be fair, the high labour turnover is not necessarily indicative of a problem with organisational strategies if this is the typical system in which the business operates. Nonetheless, labour turnover becomes a major problem when HR managers are constantly obligated to, without prior notice, rehire and train new employees for positions that the business deemed would be held for the long-term (Nickson 2013, p. 67). This is the problem that typifies the vast majority of t ourism and hospitality businesses. High labour turnover is particularly problematic in developed economies such as the UK where employees are able to acquire alternative employment when dissatisfied by the HR strategies of their present employers. [my creator] had abandoned me, and in the bitterness of my heart I cursed him,† (93-94). He is content with the knowledge that people are wanted and treated well by those who care, but is disappointed when he comprehends that he and the people around him are not one and the same. The Wretch struggles to come to terms with this, as he has not been prepared to deal with the cruelty of those who he instinctually admires. The Wretch takes offense easily, and is hardened by the fact that there is so much good in the world, yet he remains unwanted. The Wretch desperately wanted to be accepted by the cottagers, and is so overcome with grief when they reject him entirely that it oversteps his anger. He is even found to say: â€Å"I could have torn him limb from limb but my heart sunk within me as with bitter sickness, and I refrained,† (97). Sad and confused, the Wretch finds himself alone and dealing with the sudden and all-too-heavy realization that he is not wanted in the world he was brought about into. The Wretch is child-like, not unlike his first victim, and does not have an understanding of how he is received by others. The Wretch comes across William Frankenstein, Victor’s youngest brother, and wants to be his friend; but his attempts at friendship being dodged by the youngster only confuses and hurts him further. â€Å"I could seize him and educate him as my companion and friend, I should not be so desolate in this peopled earth the child still struggled, and loaded me with epithets which carried despair to my heart: I grasped his throat to silence him, and in a moment he lay dead at my feet,†(102). Regrettably, The Wretch did not realize his strength and was too taken by the idea of friendship to see what he was doing. Disappointed still at the fact that a young and impressionable child was old and wise enough to know they were different, the Wretch truly feels alone and abandoned by society. Robert Walton is a man who is always changing his mind, and changing his behaviors and focuses. He realizes a new goal for himself, to travel to the Antarctic, and sets out on yet another adventure; he is relishing in the thought that he is finally content with the direction in which his life is going. These reflections have dispelled the agitation with which I began my letter, and I feel my heart glow with an enthusiasm which elevates me to heaven; for nothing contributes so much to tranquilize the mind as a steady purposea point on which the soul may fix its intellectual eye,† (2). Walton is quoted several times to have said that he changed focuses in his life. He is inconsistent a nd fickle about his life’s goals, and never completes anything. Sure he may one day set out and finish something, but the Robert Walton depicted by Shelley and introduced to Victor Frankenstein is not that man. Robert Walton is a simple man. who is so proud, that he can not bear to be a disappointment to anyone, including himself. While writing to his sister, Walton is addressing the fact that should he fail on his latest mission, he will be far too ashamed to face that and most likely disappear completely. â€Å"If I succeed, many, many months, perhaps years, will pass before you and I may meet: If I fail, you will see me again soon, or never,† (3). From the very beginning, Robert is displayed as a character who is unstable and very easily disappointed. While this is not life ruining, a trait like this surely only complicates life and upsets those around him. In promising to estrange himself from his family solely because of a failure, one that has not even happened yet, Robert is painted as a man who perhaps should not be trusted. Walton’s emotions and how he reacts is everchanging, and he is quick to change his mind about a person or idea solely based on prejudice or the opinions of others. Robert allows himself to feel badly for the monster when listening to his grief over the death of Victor, but his sudden and ery deep obligation to Victor, as well as his prejudice against the Wretch, stops him. â€Å"I was first touched by the expressions of his misery; yet, when I called to mind what Frankenstein had said of his powers of eloquence and persuasion, and when I again cast my eyes on the lifeless form of my friend, indignation was rekindled within me,†(164). Perhaps Robert would have felt more strongly about the Wre tch and his own story had Robert not known and immediately sided with Victor. In lieu of assessing the situation and how he felt about the monster, Walton promptly writes off any good feelings for him, because he is an easily-influenced man, who is incapable of really thinking things through. Victor Frankenstein, towards the end of his life, is quick to anger when faced with even the thought of his creation. Robert Walton wanted to know what was plaguing the mind of his new friend, but was taken aback by how upset Victor was when questioned about the monster. After confronting him, Walton says, â€Å"As I spoke, a dark gloom spread over my listener’s countenance. At first I perceived that he tried to suppress his emotion; he placed his hands before his eyes, and my voice quivered and failed me as I beheld tears trickle fast from between his fingers, a groan burst from his heaving breast,† (11). Victor is a man of prestige and a scholar, to see him break down at the mention of the Wretch is largely an indicator that he is somewhat deranged. He literally breaks down and cries in front of Robert Walton, a man whom he has just been introduced to, and is so moved by his emotions that he has to excuse himself and spend the night calming himself. Though this is early on in the novel, the actual event takes place at the end of Victor’s tale, and can later be chalked up to the fact that the creation of his monster took so much out of him, that he is a different, and highly disturbed man. Victor is taken so strongly by his emotions and devotion to his project that he jeopardizes himself and his health. Victor explains the struggles he went through to create the monster, but is so enraptured with the idea of making new life, that he dismisses these downsides. He is quoted as saying, â€Å"I had worked hard for nearly two years, for the sole purpose of infusing life into an inanimate body. For this I had deprived myself of rest and health. I had desired it with an ardour that far exceeded moderation; but now that I had finished, the beauty of the dream vanished, and breathless horror and disgust filled my heart,† (35). Taken by his convictions, Victor knows what he is doing is wrong, and continues on with them anyway. He sees that his creating the monster and tampering with life is wrong, but goes through with it because his want to succeed is much greater than the battle within him over how morally right or wrong it is. As the novel progresses, this eats away at Victor, as he feels so badly about what he’s done. Victor abhors the creation he has made because he is a man who lets impressions fog his view of others. Victor himself feels a general sadness when he hears the tale of the Wretch, because not unlike Victor, nor any other â€Å"living† man, the Wretch has feelings. However, Victor openly admits that: â€Å"I compassionated him and sometimes felt a wish to console him; but when I looked upon him, when I saw the filthy mass that moved and talked, my heart sickened and my feelings were altered to those of horror and hatred,† (106). This reflects poorly on Victor, as he is the man who created the Wretch. To feel horror and hatred at one’s own creation, one whom many liken to a son of Frankenstein’s, is abominable of Victor. Victor is just a man who is incapable of looking past his preju. dices and accepting the wrongs he has done. Knowing what kind of man he is, he should not have gone through with the making of the Wretch at all. However, the drive to accomplish something great and be renown for his advancement in the science community, as well as an arguably deep-rooted want to be distinguished and intellectual, proved to be much greater than any compunction from creating life and tampering with something so delicate as the human emotion.
